Nation IN BRIEF : WASHINGTON, D.C. : Probers Fault Pilots in Fatal Heinz Crash

Poor judgment on the part of a helicopter crew and an airplane pilot probably caused the midair collision near Philadelphia that killed Sen. John Heinz and six other people in April, federal safety investigators concluded. The National Transportation Safety Board determined also that the charter company that was transporting Heinz, Lycoming Air Services Inc. of Williamsport, Pa., had inadequately trained the pilot. The board approved a report that detailed a series of questionable in-flight decisions and maneuvers by the pilots that led up to the April 4 crash.
